This plan is only honored at Sarah Pitarra, DDS located at 4758 McArdle Rd, Suite 104, Corpus Christi, TX 78411. It is not accepted or honored at any other dental office or specialist office.

This is NOT dental insurance.

Members will abide by the office policies of Sarah Pitarra, DDS. Any failure to abide by policies will result in the termination of club benefits.

There is no ID card, no group number or member number.

All membership information will be kept on file with our office.

The Sarah Pitarra, DDS Smile Membership Club is designed to provide affordability for quality dental care for the entire family. This plan cannot be combined with any insurance, special offer, or discount.

All membership services are to be paid in full at the time of service. No payment plans are offered for the Membership fees or services rendered.

Membership will be terminated for failure to pay for services rendered.
